- The name of the person being reported or sufficient identifying information
- Specific facts or evidence to support the allegation and assist in the investigation

Important Reminder:
All reports must be made in good faith. Knowingly submitting false or fabricated information is strictly prohibited. If a report is found to have been made with malicious intent or includes deliberately misleading statements, the whistleblower may be held legally responsible.
